Controlled Substance Prescription Policy

Important Information About Your Child's Controlled Substance Prescription

This letter explains our office policy for patients receiving controlled substance medications (such as stimulants for ADHD, pain medications, or certain anxiety medications).

Required Visits

To continue receiving your child's controlled substance prescription, your child must be seen in our office once every 3 months (every 90 days). These visits allow us to:

- Monitor how well the medication is working

- Check for any side effects or concerns

- Ensure the medication remains safe and appropriate for your child

- Meet state and federal requirements for controlled substance prescribing

Your Child's Annual Preventative Visit Counts

Your child's yearly preventative/well-child visit can count as one of these required quarterly visits.

Your Responsibility as a Parent/Guardian

It is your responsibility to:

- Keep track of when your child's next appointment is due

- Schedule appointments in advance to ensure your child is seen within the required timeframe

- Bring your child to all scheduled appointments

What Happens If Your Child Misses the Required Visit

If your child is not seen within the required 3-month timeframe, we will not be able to send the prescription until your child has been evaluated in the office. This policy is in place to ensure your child's safety and to comply with best practices for controlled substance prescribing.
